Data Layout Modification for Excel File

I am seeking a skilled freelancer to help modify the layout of an Excel file created from a PDF. Need data changed from a vertical name address, to a horizontal/flatter layout with the following header, Name, Address 1, Address 2, City, State, Zip, Amount. Data is found in rows 491 thru 3591.
