Responsibilities:Provide skilled nursing care, including G-tube feedings and tracheostomy careMonitor and document client health status and report changes as neededAdminister medications and treatments per physician ordersCollaborate with the healthcare and direct support teams to ensure safe, person-centered careEnsure compliance with DDI and state regulatory requirements Knowledge & AbilitiesStrong clinical assessment skills and the ability to develop and implement care plans.Excellent communication and interpersonal skills.Proficiency in managing and administering medications.Ability to work independently and as part of an interdisciplinary team.Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
