Practicing under the supervision of transplant physicians, the Transplant Nurse Coordinator provides coordination through all phases of patient care. This position makes referrals to external clinicians or treatment programs or other community resources as appropriate and in accordance with established departmental protocols. Job duties may vary by specific department or assignment.The CAR T-cell Therapy Nurse Coordinator supports patients undergoing CAR T-Cell Therapy from intake to discharge.  Responsible for coordinating CAR T-cell therapy admission for patients and caregivers, which includes facilitating insurance approval, educating patients and caregivers, coordinating harvests, coordinating pre-CAR-T work-up and evaluation, preparing for admission, and providing necessary communication and hand off to the inpatient stem cell transplant team. Unique job responsibilities include:Actively participates in weekly case conferences to facilitate the efficient transition of patients and families into the NM programPartners with Transplant Assistants to schedule all pre-CAR-T labs, testing, and proceduresReviews all testing results and communicates all abnormal results to the appropriate physicianProvides education to patients and caregivers throughout the treatment processCoordinates with outside hospitals and referring offices as neededCollaborates with Transplant Finance and Managed Care to ensure efficient insurance coordination and timely authorizations; serves as clinical liaisons to Managed Care regarding new contract language, contract renewals, and letters of agreementIn collaboration with the Clinical Trials Office, assures eligibility for inclusion into treatment and/or research protocols, educates patients on applicable clinical trials, and obtains informed consents from patients when neededEnsures compliance with manufacturer Risk Evaluation Mitigation Strategy (REMS) program requirements, including completing trainings, participating in audits, educating patients on safety measures, and ensuring operational compliance with safety measures (wallet cards, tociluzimab availability, etc.)Creates and utilizes clinical education materials for commercial CAR T-cell therapy for physicians, advanced practice professionals, nurses, social work, and other care team membersParticipates in team meetings, quality efforts, policy review, process improvement projects, and program development work as the CAR T-cell therapy program grows with the addition of new therapies and treatmentsPartners with Quality for any commercial CAR T-cell program regulatory audits and site visitsAbides by all program requirements to ensure compliance with the Foundation for the accreditation of Cellular Therapies (FACT) and FDA regulationsCollaborates with Data Team to ensure all documentation requirements for research studies are metResponsibilities:Responsible for planning, directing, providing, and documenting appropriate care for patients in the designated care settingAssists in coordinating comprehensive care for patients, including assisting with the consent process, data collection and arranging any required follow-up visits, referrals, testing, treatment, and/or long-term follow-upWorks collaboratively with the interdisciplinary team to coordinate on-going care, focused on cost-effective ways to maximize wellness and enhance patients’ livesDevelops, coordinates, and provides education for patients, family, caregivers, and staff based on personalized needs along with serving as a resource for ongoing educationServes as a communication link between the patient, family, caregivers, interdisciplinary team, and outside hospitals/agenciesContributes to departments, hospitals, or other professional committees and/or quality or process improvement initiatives.Ensures clinical practice and patient care is consistent with policies, procedures, and regulatory requirementsOther duties as assigned
