Job Title: Healthcare & Human Service Advocate (HHSA)

Division/Section/Unit: Coventry, Vernon, & Roberts, LLC (CVR)/HHSA

Job Summary:

Provides oversight and advocacy for the care, medication, treatment, placement, testing, and evaluation of CVR fiduciary “clients”, including: wards, principals, protected persons, and fiduciary contract clients. Provides CVR principals with recommendations and insight, to assist with decision-making for such clients.

Primary Duties:

Conducts analysis, prepares recommendations, and reports, as needed, in support of the coordination for placement, care, medical/mental health/dental and any other type of treatment by the correspondent practitioners. Participates in bi-weekly meetings in which principals are advised of current issues with regard to care, diagnosis, prognosis, and outcomes for each of the cases in which the HHSA is assigned. Documents incidents in accordance with the CVR standard of practice. Records discrete details of services provide to each client in a QuickBooks billing database.

Duties include a wide scope of responsibilities that include, but are not limited to:

Oversees complex case management needs and coordinates consent for treatment for medical, dental, psychiatric, personal care, transportation, and placement and coordinates service delivery.

Conducts on site visits and quarterly visits to nursing homes, group homes, supervisory care and boarding homes, client residences, medical and psychiatric hospitals, urgent care / stabilization sites, and family homes.

Completes documentation for client's social, health, and medical status, periodic visits, status of financial situation, benefits, and updates demographic information in the CVR database

Prepares for and testifies at court hearings, as needed.

Documents and recommends need for crisis intervention, behavioral issues, and unique needs - as it relates to services and placement.

Attends staff meetings and discharge staffings, as required, and documents accordingly.

Prepares reports detailing findings and recommendations for court intervention, reports of guardian, and case reviews.

Researches and documents information pertaining to treatment planning, behavioral interventions, and benefits.

Investigates referrals for guardianship and/or conservatorships, as assigned.

Interviews clients, friends, relatives, and physicians, of current or prospective clients, to obtain complete social and medical histories.

Maintains 24/7 availability (via telecommunications) for response to calls for emergency fiduciary services, in coordination with other management and staff – as determined necessary by the principals of the firm.

Qualifications/Experience/Skills/Licensure:

Requires demonstrated knowledge of the skills requisite to oversee the services required under various fiduciary roles, including: guardianship, agency under medical or mental health powers of attorney (POA), or healthcare advocacy cases. These skills include: principles and practices of health care management; ability to investigate and obtain facts; evaluate and draw appropriate conclusions; ability to manage time in order to meet numerous priorities and deadlines; ability to develop and maintain effective working relationships with others; ability to communicate openly and effectively – orally and in writing; ability to effectively work with physically and mentally incapacitated individuals and difficult familial relationships. Qualified candidates must possess a clean driving record, pass a criminal background check, and provide a satisfactory credit report. The HHSA is expected to have attained and maintain licensure as a Licensed Practical Nurse (LPN), or Registered Nurse (RN), or the academic equivalent of either credential.

Benefits & Compensation: This position is a full-time, salaried, exempt position.

Holidays: Paid holidays are as follows: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.

Insurance Coverage: Medical, Dental & Vision coverage will be available through our company's employee benefit plan for full time employees and will be effective sixty (60) days after hire date. CVR will contribute up to $425.00 per month for insurance premiums related to the CVR plans. A Simple IRA plan is offered after 24 months of employment. The company matches up to 3% contributions by the employee (base pay only). You may decline such benefits if they are not needed.

Merit Bonuses: Each year, CVR has been able to provide its employees with a bonus (generally in early December). Merit bonuses are not guaranteed and are dependent on CVR’s financial position at the time its management meets to discuss such issues.

Paid Time Off / Sick Leave Accrual: CVR offers a paid-time off plan for all full-time employees. Time off accrues at the rate of 0.019230 hours, per hour worked/paid per pay-period for the first two years of employment, up to a total of 80 hours per year. During your third year of employment, the accrual increases to a total of 120 hours per year. A portion of your accrued paid time off will be identified as "Sick Pay" on your pay stubs, as required by Arizona law.

Salary: Annual starting salary will be between $75,000.00 and $80,000.00 depending on relevant credentials and experience.

Bonuses: CVR has consistently provided an annual merit bonus prior to Christmas. This is not guaranteed. Additionally, employees are given bonuses based on exceptional service, at the recommendation of their respective supervisors.
